Case Management
Congregate and
Home Delivered Meals
The primary goal for a Case Manager is to provide senior adults the support, resources, and community services they need to safely remain in their own homes. A home visit is routinely conducted to assess individual needs. A Case Manager provides referrals and coordination of the following (but not limited to):
· Home assistance & respite care options
· Prescription & health insurance assistance
· Assessment of eligibility for federal, state, and county programs
· Caregiver Support Groups
· Elder abuse & neglect resources
· Financial & utility assistance programs
· Recreational, socialization, and volunteer programs

Office Hours are Monday - Friday from 9-3. Please call ahead to schedule an appointment as this position often requires in-home visits.

We are committed to providing nutritious meals that can enhance the independence of seniors in our community.
Participants must be over the age of 60.
Home Delivered Meals:
Served Monday– Friday between 11:00 and 1:00
Congregate Meals:
Served Monday. Wednesday and Friday at 11:30 at the
Sugar River Senior Center.
Please call the office at 424-6007 one working day ahead to make a reservation for a meal.

2020-2021 CFS Winter Closure Guidelines
Meals will be canceled for the day, if by 6:00am the National Weather Service issues one of the following warnings or advisories to go into effect prior to noon*:
-Winter Storm Warning or Advisory
-Blizzard Warning or Advisory
-Wind Chill Warning or Advisory
-Ice Storm Warning or Advisory
We will do our best to contact all meal
participants scheduled to receive a meal that day -SRSC
